As infections rise, so does the need for testing.<\/p>\n<p>Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, recently announced that health officials are considering <a href=\"https:\/\/www.washingtonpost.com\/news\/powerpost\/paloma\/the-health-202\/2020\/06\/26\/the-health-202-the-trump-administration-is-eyeing-a-new-testing-strategy-for-coronavirus-anthony-fauci-says\/5ef4f629602ff1080718f308\/\">pooled testing<\/a> for COVID-19 in response <a href=\"https:\/\/www.nytimes.com\/2020\/06\/26\/world\/coronavirus-live-updates.html#link-a741662\">to the recent surge<\/a>.<\/p>\n<p>So what is pooled testing, and why does the U.S. need it?<\/p>\n<h2>Test groups, not individuals<\/h2>\n<p>The basic idea of <a href=\"https:\/\/healthpolicy.usc.edu\/research\/getting-americans-back-to-work-and-school-with-pooled-testing\/\">pooled testing<\/a> is that it allows public health officials to test small groups \u2013 called pools \u2013 of people using only one test. This means you can test more people faster, using fewer tests and for less money.<\/p>\n<p>Instead of testing one person at a time, samples from multiple individuals would be mixed together and tested as one. If the test comes back negative, everyone in the pool is clear. If positive, each member of the pool is then tested individually.<\/p>\n<p>For example, imagine a workplace with 20 people, one of whom is infected. Pooled testing divides the 20 people into four groups of five. Samples are taken from each person and mixed with the samples from other people in their group. Each grouped sample is then tested \u2013 four tests in all.<\/p>\n<p>One of the pools will turn up positive because it contains the single infected person. All five people in this pool are then retested individually, and the one sick person is identified.<\/p>\n<p>In all, nine tests are used and all 20 workers have been screened. If everyone had been screened individually, it would have taken 20 tests.<\/p>\n<p>In practice, the number of tests saved by pooling varies <a href=\"https:\/\/healthpolicy.usc.edu\/wp-content\/uploads\/2020\/05\/USC_Schaeffer_PooledTesting_WhitePaper_FINAL-1.pdf\">depends on the fraction of people infected<\/a>. Higher infection rates mean that more pools come back positive, more people need to be retested, and savings from pooling are lower.<\/p>\n<p>For example, pooled testing on suspected COVID-19 cases in Los Angeles \u2013 where as of June 27 <a href=\"https:\/\/www.latimes.com\/california\/story\/2020-06-27\/rate-of-coronavirus-test-results-confirming-infection-rises-sharply-in-california\">nearly 9% of people tested are positive<\/a> \u2013 would reduce test use by about 50%. State labs have used it <a href=\"https:\/\/doi.org\/10.1002\/sim.5334\">to screen for chlamydia and gonorrhea<\/a> and the Red Cross has used it <a href=\"https:\/\/www.redcrossblood.org\/biomedical-services\/blood-diagnostic-testing\/blood-testing.html\">to test donated blood<\/a> for Hepatitis B and C, Zika virus and HIV.<\/p>\n<p>And some places have already begun using pooled testing to screen for active coronavirus infections.<\/p>\n<p>[<em>The Conversation\u2019s newsletter explains what\u2019s going on with the coronavirus pandemic. <a href=\"https:\/\/theconversation.com\/us\/newsletters\/the-daily-3?utm_source=TCUS&amp;utm_medium=inline-link&amp;utm_campaign=newsletter-text&amp;utm_content=coronavirus-going-on\">Subscribe now<\/a>.<\/em>]<\/p>\n<p>Stanford University <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2020\/04\/testing-pooled-samples-to-track-early-spread-of-virus.html\">used pooled testing<\/a> to monitor the early spread of COVID-19 in the Bay Area. Nebraska\u2019s Public Health Laboratory <a href=\"https:\/\/dx.doi.org\/10.1093%2Fajcp%2Faqaa064\">pooled and tested samples from five-person groups<\/a> and <a href=\"https:\/\/www.nytimes.com\/2020\/05\/07\/opinion\/coronavirus-group-testing.html\">roughly doubled<\/a> the number of suspected COVID-19 patients it could test weekly.<\/p>\n<p>Some people have expressed concern that the dilution of samples from pooling could reduce the accuracy of the PCR tests that look for viral RNA in samples. Fortunately, researchers have already shown that pooled testing is about as accurate as individual testing <a href=\"https:\/\/doi.org\/10.1016\/j.cmi.2020.06.009\">in pools as large as eight people<\/a>.<\/p>\n<p>Another concern is infection prevalence. When infection rates <a href=\"https:\/\/healthpolicy.usc.edu\/research\/getting-americans-back-to-work-and-school-with-pooled-testing\/\">climb higher than 15%<\/a>, too many pools come back positive and pooling is no longer more efficient. Fortunately, on a nationwide basis only about <a href=\"https:\/\/coronavirus.jhu.edu\/testing\/individual-states\">5% of tests<\/a> on suspected cases are coming back positive. Even in extreme hot spots like Texas, where <a href=\"https:\/\/www.texastribune.org\/2020\/06\/24\/texas-coronavirus-infection-rate\/\">10% or more of suspected cases are testing positive<\/a>, pooled testing could still be useful.<\/p>\n<p>But the real strength of pooled testing would be seen if health officials start implementing large-scale testing of the general population.<\/p>\n<p>For the most part, patients are currently tested only when a health care provider <a href=\"https:\/\/www.fda.gov\/medical-devices\/emergency-situations-medical-devices\/faqs-testing-sars-cov-2\">suspects they might be infected with SARS-CoV-2<\/a>. However, <a href=\"https:\/\/theconversation.com\/can-people-spread-the-coronavirus-if-they-dont-have-symptoms-5-questions-answered-about-asymptomatic-covid-19-140531\">asymptomatic and presymptomatic COVID-19 patients are common<\/a> and are likely a <a href=\"https:\/\/doi.org\/10.1126\/science.abb6936\">major source of COVID-19 transmission<\/a>. For this reason, <a href=\"https:\/\/dx.doi.org\/10.1007%2Fs40258-020-00579-4\">experts have recommended<\/a> screening everyone for infections.<\/p>\n<p>In the general population, the prevalence of new infections is estimated to be <a href=\"https:\/\/www.cdc.gov\/covid-data-tracker\/#cases\">less than 2%<\/a>, even in the hardest-hit states. Yet, some experts have called for testing America\u2019s entire population <a href=\"https:\/\/www.rockefellerfoundation.org\/national-covid-19-testing-action-plan\/\">every three months<\/a> or even <a href=\"https:\/\/paulromer.net\/covid-sim-part2\/\">every two weeks<\/a>.<\/p>\n<p>Considering that American labs are <a href=\"https:\/\/www.medtechdive.com\/news\/widespread-swab-covid-19-test-materials-shortages-persist-poll-says\/578831\/\">already short of testing reagents<\/a>, it seems doubtful companies could soon produce enough material to run this many individual tests \u2013 assuming companies or governments could even pay for them all. Widespread use of pooled testing can help screen more people using fewer laboratory tests.<\/p>\n<p>In order to increase capacity, the FDA recently released guidance <a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/coronavirus-covid-19-update-facilitating-diagnostic-test-availability-asymptomatic-testing-and\">that lays out a path to pooled testing authorization<\/a> so more labs and test manufacturers can start doing it.<\/p>\n<p>Another potential obstacle is cost. If labs charge much more for testing a pooled sample than they do for an individual one, any cost savings will dissipate.<\/p>\n<p>Cheaper, faster testing methods enable screening that is more widespread and more frequent. Frequency is critical for monitoring a disease that spreads rapidly. Moreover, repeat testing helps pick up cases that might have been missed previously. Retesting helps, because research suggests that PCR tests miss <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/tests-may-miss-more-than-1-in-5-covid-19-cases\">about 20%<\/a> of infected cases, no matter whether they are pooled or not.<\/p>\n<p>Pooling could get the U.S. one step closer to the goal of universal and repeated testing \u2013 and play a crucial role in containing the spread of this relentless disease.<!-- Below is The Conversation's page counter tag.
